A 29-year old woman presented with diffuse swelling of the base of the right thumb along with ulceration. X-ray indicated bony damage. Histopathology showed PAS positive intracellular organisms suggestive of histoplasmosis. We are reporting a very rare case of primary cutaneous histoplasmosis from this part of the country.

Evaluation of patch test in identification of causative agent in drug rashes due to antiepileptics

Patch test was evaluated for the identification of causative agent in cutaneous eruptions due to antiepileptics. Patch tests were carried out in twenty patients and ten controls with carbamazepine, phenytoin sodium, phenobarbitone and sodium valproate. Sodium valproate was found tobe irritant in 1 and 5% concentration and further dilution is recommended for patch testing. Patch test was positive in 14 (70%) patients and in 7 with suspected drug alone, and remaining 7 were positive with more than one antiepileptic drug. We recommended patch test for identification of causative drug in rashes due to antiepileptics.

Increased Lipid Peroxidation in Lepromatous Leprosy

The biochemical marker of lipid peroxidation, malondialdehyde (MDA) was measured in plasma of 70 leprosy patients. Erythrocyte superoxide dismutase activity (SOD) was also measured in the patients and the values were compared with 15 normal healthy controls. Mean plasma MDA level was lowest (3.225 ± 0.24) in tuberculoid leprosy (TT), while it was highest (4.516 ± 0.54) in lepromatous leprosy (LL). Whereas, SOD activity was highest in TT (34.05 ± 2.46) and it was lowest (25.84 ± 5.67 units/mg Hb) in LL. The ratio of MDA/SOD as the blood index of oxidative stress was also determined. It was found that mean ratio was lowest in TT (0.095) while it was highest (0.185) in LL. These results strongly indicate increased oxidative stress in LL which appears to have resulted due to low antioxidant defence in LL.

An analysis of relapsed leprosy cases

An analysis of relapsed leprosy cases registered in the urban leprosy centre attached to the department of Dermatology and Venereology over the last 5 years was carried out. The characteristics of these cases including the clinical features, the types of treatment given and its duration were studied. The percentage of relapse for this period was 3.88% with paucibacillary cases showing more susceptibility for relapse (5.4%) than multibacillary cases (2.15%). The percentage of relapse increased year by year. Prolonged treatment and even multibacillary regime in paucibacillary cases did not prevent relapse. Hence relapse may be inherent in certain cases of leprosy which is unaffected by extraneous factors like type of treatment and its duration.

Dapsone syndrome or sulphone syndrome was noticed within four to six weeks of starting treatment in 10 out of 604 patients (1.6%) on MDT for leprosy treated at Rajah Muthiah Medical College Hospital, South Arcot District, Tamil Nadu State during the period 1995 - 1998. Patients developed either maculo papular rash or exfoliation along with fever and lymphadenopathy. Abnormal liver function tests was noticed in 50%. The patients with dapsone syndrome were treated with corticosteroids after withdrawing dapsone. There was complete resolution of skin lesion and other symptoms.

Failure of pentoxifylline to affect air- borne contact dermatitis caused by parthenium

Twelve patients, 9 males and 3 females between 22 to 65 years in age having air-borne contact dermatitis (ABCD) for 6 months to 12 years were included in this study. The diagnosis in each patient was confirmed by patch tests. Six of these patients (group I) were treated with pentoxifylline 400 mg three times a day orally in addition to topical clobetasol propionate and oral betamethasone 1.0 to 2.0 mg per day for 5 to 10 weeks. The other 6 patients (group II) were given the same treatment for the same duration but without pentoxifylline. The response to the treatment in both the groups was almost the same. The patients treated with pentoxifylline did not have any added benefit. There were no significant side effects of the therapy in either of the groups.
