Multiple Keratocanthomas on the Mons Pubis and Labia Majora

An ederly woman developed multiple keratoacanthomas on the mons pubic and labia majora. Some lesion coalesced to from large irregular plaques with multiple craters. Biopsy of a well developed nodule revealed keratin - filled craters, pseudoepitheliomatous hyperplasia with horn pearls and dyskeratosis of prickle cells which appeared eosinophilio and glassy. Though a few lesions involuted spontaneously, some persisted. These responded well to topical 5-fluorouracil therapy. There was no recurrence of lesions, on long follow-up

Vitiligo among the community Somavashan Sahasajuna Kashatriya(SSK) of Baugalore surveyed by us during February-March, 1987, was found to be characterised by an earlier age of onset and a very high (90.38%) familial incidence. This community at present is small and is believed to have migrated'from Gujarat-Maharashtra border where the incidence of vitiligo in a related community, the Khatris is already high (3.6%). The results indicate that continuous inbreeding among the SSK community in Bangalore may have enhanced their genetic predisposition to this disease.

A 33 year old male had severely itchy psonasiform lesions on his scalp, face, arms and shaft of penis. He also had loss of hair on the lateral half. of both eyebrows and the temporal. part of his scalp, generalized lymphadenopathy, coppery coloured macules on his pahns and soles, ulcers on the mu,cosa of his upper lip and soft palate and condyloma lata on his anal mucosa. VDRL test was positive in the titre of 1: 128. The itching along with the other symptoms and signs disappeared completely in one mouth with a single dose of 2.4 mega units of benezathine pencillin given inramuscularly.

Langerhans cells (LC) were defined by indirect immunofluoresence using OKT6 monoclonal antibody in the skin lesions of 11 untreated patients with tinea corporis. The number and distribution of OKr6 + epidermal LC in the lesions was very low in comparison to that found in the normal skin. In most of the lesions, T6 + LC could not be seen in the epidermis. A like antigens were not detectable on the keratinocytes. These observations suggest deceased CMI response to the fungal infection and that the reduction in LC may be a contributing factor in the pathogenesis of tinea corporis.

Summer Variant of Autosmal Dominant Ichthyosis Vulgaris

Eleven cases of mild autosomal dominant ichthyosis vulgaris (ADIV) were seen who presented with asymptomatic, brownish-black hyper keratotic lesions' over the flexure surfaces of wrists, dorsum of hands, knees and ankles for 1-2 months during peak summer months. The lesions disappeared with the onset of monsoon. All these cases had typical lesions of ADIV during winter months. This paradoxical phenomenon of ichthyosis alternating with hyperkeratotic pigmented lesions in the same individual during different seasons remains unexplained. Treatment with 20% urea cream and oral vitamin A had no effect on the course of the disease.

Nuts As a Cause of Contact Dermatitis of the Hands

To evaluate contact hypersensitivity to some of the commonly used Indian nuts, patch tests were undertaken with cashew nuts, ground nuts, almonds and pistachio in 3 groups of individuals: (1) patients exposed to the corresponding nut and having dermatitis on their fingertips, (2) controls having no dermatitis, but exposed to the nut, and (3) unexposed group. A paste prepared by Cushing the nut and used as such gave positive patch test 'reactions in, all the 6 patients tested with cashew - nut, ground - nut and almonds, and these looked to be irritant reactions. Further patch tests with these three nuts were therefore done with 1:1 dilution of the paste with distilled water. The number of patients showing positive patch tests out of the total number

Cutaneous endometriosis is a well recognized but uncommon phenomenon. We observed a case of this condition occurring in an abdominal hysterectomy scar 6 months after the operation. The nodule developed pain once a month. Histopathology revealed glandular structures lined by a single layer of columnar cells. Surgical ecision resulted in cure of the condition.

Humoral Immuno-deficiency in Lichen Planus

Serum immunouglobulin levels were studied in 41 histopathologically proven idiopathic lichen planus patients and 25 normal healthy individuals. No difference was observed in IgG levels, though, IgA and IgM levels were significantly reduced 'm patients with lichen planus as compared to normal controls. There was no association between the mucosal involvement and the reduced levels of IgA and IgM. These results suggest the possibility of humoral immuno-deficiency, in lichen planus patients.

Radio-dermatitis developed at the site of X-ray irradiation received 35 year ago for tuberculosis of the cervical lymph glands. Five cases of porokeratosis are reported. Two of these were of the classical mibelli type,in one case only one segment of the limb was involved in a linear fashion, in the fourth case a linear lesion involved the entire left side of the body, and the fifth case was of the disseminated superficial type. Typical histopathological features were observed in porokeratosis of Mibelli and also in the case of disseminated superficial type. In one case short contact dithranol and isotretinoic acid treatment was successful.
