Methotrexate: Side effects and the role of folic acid supplementation in psoriasis - A study

In 50 patients of psoriasis, side effects observed with methotrexate pulse were studied. Folic acid ameliorated majority of the side effects without compromising with the therapeutic efficacy of methotrexate.

This Kap (Knowledge, Attitude, Practical) study was to assess the current level of knowledge among first year M.B.B.S. students about AIDS at the point of entry to medical course. One hundred and seventy-one students were given a pre-tested close ended questionnaire. Overall level of knowledge about AIDS was found to be 64.91%. Male students were found to have better knowledge regarding transmission and prevention of AIDS than female students. Misconceptions were found to exist regarding mode of transmission, prognosis and prevention. Awareness programmes should be initiated among M.B.B.S. students in the first year itself, so that, lacunae can be rectified.

Cutaneous manifestations of diabetes mellitus a clinical study

Eighty-eight patients including 64 males and 24 females with diabetes mellitus and skin diseases were studied. The skin manifestations were common during the fifth decade of life. Maturity onset diabetes had increased incidence of skin disease. Incidence of skin disease was common in early diabetes. Infections both bacterial and fungal were seen in majority of patients. Specific cutaneous markers and metabolic changes were rare and seen in long standing diabetes. Infections were the common problems which recurred subsequently.

Radiological changes in psoriatic arthropathy

Forty-one cases of psoriatic arthropathy (PA) were selected for the study. Biopsy, X rays of hands, feet, cervical and dorsolumbar spine, sacro-iliac joints and routine investigations were carried out. Clinical diagnosis of psoriasis was confirmed histopathologically. Radiological changes in order of frequency were most common in feet - 26/41, hands - 24/41, sacro-iliac joints - 11/41, dorso-lumbar spine - 4/41 and cervical spine-3/41. Distinctive radiological changes were seen in psoriatic arthritis.

Epidermodysplasia verruciformis

A 14-year-old female presented with multiple hypopigmented macules and papules over trunk, face and neck of 10 years duration. She also had verrucous plaques over the extremities. Biopsy of the lesion was consistent with the diagnosis of epidermodysplasia verruciformis. Some of the lesions flattened following placentrex injections. At present, the patient does not have any malignant or premalignant lesion and she is under observation.

Paediatric HIV infection

Four cases of HIV infection in children between one to eight years of age are reported Three were males and the other one female. One child was born to known HIV infected mother. The female child was victim of sexual abuse and had disseminated tuberculosis with syphilis and gonorrhoea. Other two children were having systemic as well as dermatological disorders for which HIV test was done and found positive.

Mycetoma on an unusual site

A 23-year-old man had a large, painless swelling on upper part of back. Swelling was studded with multiple nodules, abscesses, and discharging and dried up sinuses. Histopathology revealed features of mycetoma. Culture yielded growth of Madurella mycetomatis.

Localised cutaneous blastomycosis

A case of localized cutaneous blastomycosis is reported for its rarity from Indian Subcontinent. This patient had distinctive morphology of an annular verrucous lesion, resembling a “three quarter moon”. The lesion regressed remarkably following ketoconazole therapy.

A 48-year-old female presented with multiple yellow coloured papules, nodules and plaques over both hands, forearms, elbows, gluteal region and thighs. Investigation revealed very high cholesterol levels in blood and low T3 level. Skin biopsy showed xanthoma cells. T3 level increased after eltroxin therapy.

A 41-year-old man with larva currens was diagnosed to have HTV infection. There was no evidence of hyperinfection with Strongyloses stercoralis. The effect of coexistent HTV infection in the natural course of strongyloidiasis is discussed.
