In this study we retrospectively analysed the unique efficacy and incidence of potentially significant side-effects of a new drug which has claimed to be a major milestone in dermatotherapeutics-isotretnoin. We concluded that oral retinoids 'do things' that nothing else can in several skin disorders-especially recalcitrant acne vulgaris which can emotionally, physically and psychologically cripple an individual for life.

A 5.5-year-old male presented with asymptomatic nodules and plaques on his scalp and pubic region of 2 months' duration. He was having productive cough, haemoptysis, chest pain, anorexia and weight loss and receiving antitubercular treatment for these symptoms for last 3 months. Clinical diagnosis of cutaneous metastatic disease was made. Chest x-ray revealed multiple coin shaped shadows on both sides with pleural effusion. Routine investigations were normal except for anemia and hyperuricemia. Biopsy of skin nodules showed features of metastatic adenocarcinoma. Features and significance of cutaneous metastases are discussed.

Atopic dermatitis is a difficult disease to treat. A few reports suggest that azathioprine may be effective in the treatment of atopic dermatitis. We therefore, evaluated the role of azathioprine in adult atopic dermatitis not adequately controlled with conventional therapy. Four patients of adult atopic dermatitis were treated with azathioprine 50 mg twice daily orally for 3-4 months. All the patients had complete remission after therapy. There were no significant side effects. Therefore, aeathioprinc can be considered a better and safer alternative drug in recalcitrant adult atopic dermatitis.

Clinical and histopathological features of epidemic dropsy were studied in 19 patients. Bilateral pitting pedal oedema, erythrocyanosis and tachycardia without fever were the predominant clinical features. Histology revealed deposition of hyaline material on the walls of dermal blood vessels.

A clinics-bacteriological study of 52 patients with urethritis was carried out. All the patients were evaluated clinically and bacteriologically at the beginning and end of the treatment. Majority of the patients belonged to the age group 21-30 years (58%). Incidence of gonococcal urethritis was 65% and Non gonococcal Urethritis (NGU) 35%. The common organisms causing NGU were chlamydia (28%) ureaplasma (11%) and mycoplasma (11%). Neisscria gonorrhoea was resistant to penicillin in (38%), ciprolloxacin in 67, and to noriloxaein in 6% cases. High incidence of HIV positivity was found in gonorrhoea (21%).

A 40-year-old promiscuous man presented with nodulo ulcerative lesions all over the body and a healing genital ulcer. Blood VDRL was reactive in 64 dilutions and HIV (Elisa) was positive. Patient was diagnosed to have malignant syphilis (leas inaligna) and was given appropriate treatment. Lesions healed with hypopigmented macules suggestive of 'Icukoderma colli'.

Fourteen cases of sub acute cutaneous lupus erythematosus (SCLE) were selected from Dermato- Venereology outpatients during the last 2½ years. Clinically all patients revealed photosensitivity and annular plaques either covered with peripheral collarette of scale or EM - like or DLE - like lesions. Systemic associations were arthralgia in 4, hypertension in I. rheumatoid arthritis in I and pulmonary tuberculosis in L Histopathologically epidermal atrophy, interface dermatitis, basal cell degeneration, colloid bodies and mononuclear infiltrate of dermis were salient features. Good response to 15 mg prednisolonc, medium potency topical steroids and sunscreens was seen in all cases.

Two female siblings of a family presented with a mechanobullous disorder since birth. Both had retarded physical development, flaccid bullac, extensive cutaneous erosions and scars, mucosal erosions, milia, corneal haziness, deformed and carious teeth, dystrophic nails, cicatricial alopecia of scalp and positive Nikolsky's sign. Skin biopsy revealed subepidermal bulla. They were diagnosed as generalized recessive dystrophic epidermolysis bbllosa. They were given oral phcnytoin but failed to show significant response.

Four cases of Leser-Trelat sign were studied in detail clinically and were investigated to establish their association with internal malignancy. One of them had papillary carcinoma of thyroid and the second had carcinoma of the body of pancreas. USG revealed mass in the gall bladder in the third case. He was lost before completion of other investigations. The fourth case was normal. So patients with Leser-Trclat sign must be carefully investigated in detail.

Cicatricial pemphigoid (CP) occurring in a 60-year-old male is described. He had involvement of conjunctival, nasal and oral mucosae, in addition to skin lesions. Diagnosis was confirmed by histopathology. He responded well to oral steroids and dapsone. However, vision in one eye was lost.

A 31-year-old man had asymptomatic, stationary, 1.5X2 cm, shiny, smooth, dark blue nodule on dorsum of right hand since 12-14 years. In addition he had developed extensive eruption of yellow to orange papulonodular lesions on extensors of limbs and buttocks since one and half months. Investigations confirmed that yellow papules were xanthomatosis and he had associated diabetes mellitus and hyperlipidaemia. Biopsy of blue nodule confirmed the clinical diagnosis of cellular blue naevus. Cellular blue naevus is rare and its association with xanthomatosis and diabetes mellitus were interesting features of above patients which is being reported for its rarity.

A 2-year-old mare child presented with multiple hypopigmented, flat topped papules with a bilateral distribution along the lines of Blaschko. A 7-year-old girl presented with multiple hypopigmented macules arranged linearly along the left upper and lower limb and also on the back along the lines of Blaschko.
