A ten year study of STD Cases in an urban clinic in Calcutta

Total 16440 patients attended the STD clinic during the 10 years period of study from 1984 to 1993. From 1988 number of STD caes were gradually decreasing probably due to less promiscuity in fear of AIDS and different measures taken to prevent transmission of HIV infection. But it does not lessen the importance of STD control, because syphilis is still prevalent (8%) with congenital syphilis. Peak in the incidence of chancroid (15%) is alarming as this may lead to increased transmission of HIV infection in near future. Male unmarried constituted the bulk of STD sufferers (44%) and married males (34%), while female unmarried and married patients were 1% and 20% respectively, 5.7% of antenatal mothers were strongly seroreactive for syphilis. Therefore all antenatal mothers should be screened for STD and routine serological test for syphilis should be done.

Patch photopatch test at Manipal

Patch and photopatch testing was performed on 55 patients with history of photosensitivity using Scandanavian photo patch test antigens obtained from Chemotechnique Diagnostics AB Sweden. The commonest reactions were seen to perfume mix 4 (21.0%), PABA 3 (15.78%), promethazine hydrochloride 3 (15.78%), chlorpromazine hydrochloride 3 (15.78%), balsam of peru 2 (10.52%), usnic acid, hexachlorophane, musk ambrette and 6 methyl coumarin showed 1 positive reaction each (5.26%) suggesting either phototoxicity or photo sensitization. Patch and photo patch test positive reaction suggesting allergic sensitisation was seen to balsam of peru 3 (23.0%) perfume mix 3 (23.0%) promethazine hydrochloride 2 (15.3%) and PABA, 6 methyl coumarin, tribromosalicylanilide, atranorin and wood mix showed positive reaction in one case each (7.69%). We conclude that photoxic or photo allergic reaction is a problem in India and patch photo patch test should be performed in all cases of idiopathic light eruptions to rule out photo sensitisation and in cases where photo sensitivity of exogenous origin is suspected.

Perforation of the palate in secondary syphilis

A middle aged woman with secondary syphilis-diagnosed clinically and serologically was prescribed oral tetracyline. She did not complete the course of treatment. Recurrent mucosal erosions in the mouth she developed subsequently as manifestation of relapsing secondary syphilis were wrongly diagnosed as 'aphthous stomatitis' and were treated with betamethasone gargling. The erosions progressed to ulcerations and finally resulted in perforation of the soft palate.

Minocycline versus co-trimoxazole in chancroid : A double-blind randomised study

This double-blind randomised parallel-group study comparing the efficacy and side effects of minocycline with that of cotrimoxazole in chancroid, had 56 analysable cases, 28 in each group. All admissible cases were assessed clinically on a scale of 0 to 3 for number and size of ulcers, pain, discharge, surrounding erythema and bubo. Each drug individually showed significant improvement in all clinical parameters. Minocycline showed significantly better improvement than cotrimoxazole in all parameters. Minocycline had 43% cure rate, and no failures, against 36% cure and 25% failure for cotrimoxazole. Both the drug were well tolerated. We conclude that minocycline is a superior alternative to cotrimoxazole in the therapy of chancroid.

Lupus erythematosus panniculitis

An infant presented with thick erythematous plaques having central necrosis, situated over cheeks and wrists. On the basis of clinical features and histology, diagnosis of lupus erythematosus panniculitis (LEP) was made. Lesions responded well to systemic steroids. To our knowledge, cases of LEP in this age group have not been reported earlier.
