Cutaneous Manifestations of Chronic Renal Failure

Cutaneous manifestations of chronic renal failure (CRF) were studied in 20 cases. Generalised pruritus was observed in 8 patients, ichthyotic skin changes of varying degree in 18 patients and hyperpigmentation of sun-exposed skin in 7 patients. Scaling, pallor and hair loss correlated strongly with the severity of CRF. Skin biopsy performed in 5 cases revealed mild hyperkeratosis, reduction of prickle cell layer and flattening of rate ridges. No significant dermal changes were detected.

Coexistent Bullous Darier's Disease with Xeroderma Pigmentosum

A case of bullous Darier's disease had typical brown follicular papules, greasy crusted papules discrete keratotic papules and vesiculo- bullous lesions on the dorsum of the hands and feet. In addition, characteristic lesions of xeroderma pigmentosum as extensivee, freekles intermingled with atrophic white, spots were present on the, face, trunk and limbs. This combination of bullous Darier's diseaese with xeroderma pigmentosum has not been reported earlier.

Correlation of Circulating Antibody Titres with Activity of Disease in Pemphigus

Correlation of antibody tires with the clinical severity of the disease was done in 58 patients having pemphigus. Serial determinations of the antibody titre-s were done in 40 patients ranging from two to eight follow-up samples. Titre of the antibody correlated with the cases. In 6 disease, activity in 70% of the cases. In 6 patients with mild disease, the antibody titre was persistently high, while in 11 patients with moderate to severe disease activity, antibody was nil or present in - low- titres. While the former group may indicate better prognosis and prolonged remissions the latter situation could be as a result of immune failure either due to fulmlinant disease or due to the immunosuppressive effect of the, high doses of the drugs used to control the disease.

Nystatin in Psoriasis

The hypothesis of the role of Candida in the gut for provoking psoriasis was tested by treating 9 males and 6 females having stable psoriasis with 200,000 units of nystatin orally 4 times a day for a minimum of 6 weeks. Intradermal candidin test and throat swab and stool samples for culture of Candida were taken before and twice after (between 4 and 8 weeks) institution of the therapy. Various Candida species isolated before therapy in 11 patients, disappeared after the therapy in 6. In the other 5 the colony counts came down. Only scaling became less in 4 patients. More than 50% clearance was noted in 2 patients. No improvement occurred in 9 patients. No obivous correlation between isolation and disappearance of Candida and persistence or clearance of lesions was observed. Immediate (Type-1) hypersensitivity response to candidin was positive in only 3 patients. Immediate hypersensitivity to Candida antigens -or its metabolic products does not appear to have any role in the pathogenesis of psoriasis.

Aetiological Spectrum of Urticaria

One hundred patients of urticaria were investigated to establish the causative agents. ,A detailed hist,6ry,:ccal examination and routine investigations led to detection of the cause m O/. patie s. Bacterial foci were responsible in 10%, worm infestations in 6%, drugs in 6%, insect bites in 3%, cold in 2%, chofine 'c in 4% and demographic in 3% rgl cases. In 60% cases, the cause was detected by intradermal tests with antigen extracts of pouen- hm fragments, dust, danders and food. Inhalantt were responsible in 35%: and foods in 25% cases. Among inhalants, pollen were found in 26%, insects in 1%, fungi in 9%, house dust and buffalo dander 'm 10/c each. In 6%, patients, no cause could be detected it; of extensive investigation.

Acne Fulminans Treated with Oral Retinoids

A 22 year old male patient suffering for 4 year from severe nodulo-cystic acne and scarring by systemic signs like fever, malaise, polyarthralgia, loss of; weight and leucocytosis was diagnosed as a case of acne fulminans. When there was no response to routine, treatment and even systemic corticosteroid, he-was treated - with a low dose of oral retinoid, isotretinoin. He improved dramatically within 2 weeks, and there was no relapse after 18 weeks follow - UP.

Delusion of Parasitosis

Delusion of parasitosis is a type of mono-symptomatic hypo-chondriacal psychosis common in elderly females whose response to drug therapy, usually pimozide, is variable. Two male patients presented with the delusion on their skin being infested with minute insects. One of them experienced presence of the inside the body cavities and spreading all over the body. They were treated satisfactorily with the neuroleptic drug, trifluoperazine.
