Topical Human Placental Extract For the Treatment of Vitiligo

Fifty percent human placental hydroalcoholic extract was used topically on 34 vitiligo patients. Complete clearing of lesions was seen in 20.6% cases, whereas 50% cases showed mild to moderate improvement, and 29.4% showed no response. No serious side effects were seen in any of the patients.

Rapidly Progressive (crescentric) Glomerulonephritis in Erythema Nodosum Leprosum

A middle aged man was hospitalised in the state of acute renal failure with erythema nodosum lepromum. He was having progressive anaemia, oliguria, azotemia and impaired renal function. The outcome was fatal. Autopsy revealed smooth congested kidney with histopathological features of rapidly progressive - (crescentric) glomerulo-nephritis, presumably a result of immune-complex deposition from recurrent erythema nodosum leprosum episodes.

Eosinophilic Spongiosis in Vesiculo-bullous Diseases

A retrospective review of the skin biopsies of 93 patients having various vesiculo-bullous diseases seen between 1983 and 1985 revealed eosinophilic spongiosis (ES) in 31 cases. of these, 19 (36.5%) out of 52 patients having pemphigus vulgaris 2(20%) of 10 patiens having pemphigus foliaceous, one patient having pemphigus vegetans, 3 (15%) of 20 patients with bullous pemphigoid, and 3 (50%) of 6 patients with dermatitis herpetiformis had ES. The severity of eosinophilic spongiosis graded on an arbitrary scale, was 3 + in 2 cases of pemphigus vulgaris, 2 + in 7 cases of pemphigus vulgaris, 1 case of pemphigus vegetans and 2 cases of dermatitis herpetiformis, while in the remaining it was only 1 +. Only 3 patients of pemphigus vulgaris showed distinctive atypical clinical features in the form of erythema multiforme like lesions in 2 cases and erythema annulare centrifugum like lesions in 1 case. In the remaining cases the clinical presentation was not different from those who did not have ES.

Exacerbation of Dermatitis Herpetiformis by Hen’s Egg and Fluoride Tooth Paste

Exacerbation of skin lesions of dermatitis herpetiformis occurred after intake of hen's eggs in a young female and after use of fluoride containing tooth paste in another patient. Elimination of eggs from the diet in the first case and evidence of fluoride containing tooth paste in the second case resulted in remission of the skin lesions.

Staphylococcal Botryomycosis of the Glans Penis

A middle aged male had botryomycosis affecting the glans penis. There were macroscopic granules that on Gram staining showed masses of staphylococci.Staphylococcus aureus was isolated from the lesion. There was prompt response to surgical removal of the affected tissue in conjunction with systemic cloxacillin.

Adenoid Cystic Carcinoma of the Skin

Adenoid cystic carcinoma commonly arises from the major and minor salivary glands. Primary cutaneous adenoid cystic carcinoma is extremely rare. The tumour was hard, non-tender, and fixed to the underlying stucture with visible congested veins. Histopathologically, large cell masses with adenoid or cribriform pattern, forming cystic spaces at places were seen. Solid epithelial nests and a few ductal structures were also seen. Plumonary metastases were another unusual feature in our patient perhaps not reported before.

Daspsone Induced Peripheral Neuropathy

A 24 year old lady being treated with 300 mg of dapsone daily for dermatitits herpetiformis, developed weakness and wasting of muscles of feet with claw hand deformity and t drop, 2 months tater. Neurological examination and nerve conduction studies conformed the presence of a peripheral motor neuropathy. Dapsone was discontinued and the patient was treated with cotrimatoxazole, gluten-free diet and supportive therapy. This satisfactorily controlled the dermatological lesion without adversely affecting the resolution of her neuropthy. Symptomatic improvement reported by the patient was confirmed by EMG and nerve conduction studies.
