Treatment of Lichen Planus with Propranalol

Accidental observation of early cure of lichen planus in a female patient who received propranalol for the treatment of associated hyperthyroidism made us to try this drug in 20 patients with lichen planus. The cutaneous lesions responded very well to the treatment in 909'o of the cases. The mucous membrane lesions responded only partially and slowly. Another group of 20 patients with lichen planus who received only oral pheniramine maleate showed persistence of skin lesions even after 3 months. There were no serious side effects -with propranalol, though one patient with an atopic background, developed bronchial asthma for the first time at the age of 21. The excellent therapeutic response to propranalol-a beta-adrenergic receptor blocker, in these patients indicates further study.

Evaluation of Efficacy of Different Topical Corticosteroids

Four different, proprietary preparations of topical cortico-steroids in cream base were evaluated for their relative efficacy on forty adult human volunteers using a modification of the method described by Reddy and Singh. Fluocinolone acetonide 0.025% (Flucort) was rated to be the most efficacious, followed by halcinonide 0.1%, (Halcinonide), beclomethasone dipropionate 0.025% (Beciate) and betamethasone-- valerate 0.12% (Betnovate). However, the differences between these four topical corticosteroids did. not reach the level of statistical significance.

Dowling Degos Disease

Fifteen members of a large family were known to be affected by a reticulate pigmentary anomaly, chiefly of the flexures of whom five members were examined and found to be affected by Dowling Degos d:sease. The family tree supports autosomal dominant transmission with female preponderance. The unusual features noted include distribution of the lesions to the acral parts, palmar pigmentation and seborrhoeic keratoses like lesions.

Age-at-onset of Leprosy

In a retrospective analysis of 971 patients, the mean age at onset of the is recorded to be 24.1 years. Sex had no, influence on the age-at-onset of leprosy. However, a atatistically significant variation was noted between the type of leprosy and, the at-onset, it was the lowest (19.2 years) in indeterminate and highest (31.08 years) in neuritic leprosy. Our . ob serrations lend support to the prevailing concept that the ageat-onset of leprosy not only varies from one country to another but also indifferent parts of the same country.

Persistence of Psoralen Repigmentation in Vitiligo

Though psoralens are successful in the treatment of vitiligo, especially in the coloured races, persistence of the repigmentation after stoppmg the drug has not en studied in detail. Opinion is, divided regarding the maintenance therapy. Among 16 vitiligo patients treated with oral psoralens, new lesions appeared in 6, though r6pigmentation obtained with these drugs remained stable. There was partial loss of repiginentation in 5 patients, complete loss in 2 complete loss and new lesions in one. Only two patients showed no change upto 2 years.

Two cases of epidermolytic hyperkeratosis are described, one having a generalised variety along with mental retardation, nystagmus, unilateral breast atrophy and intermittent divergent aquint, and the other having the localised variety associated with mental retardation, ventricular septal defect, sensorineural deafness, hypoplasia of first left metacarpal and extensive dental caries.

Persistent Scabious Nodules (a Clinico-pathologic Study)

Twenty-nine (5.33%) out of 544 patients of scabies had persistent smbious nodules. Young adults (16-30 years) and children under 15 years constituted 48.27% and 34.48% patients respectively. All but one were males, scrotum (96.55%) and penis (69.0%) were the commonest sites affected, followed by axillaiy region (37.93%) and groins (17.24%). Nodules disappeared in all the patients within 4 months. Histopathlogical studies done in 11 patients showed intracellular or intercellular oedema, occasionally distinct vesicle formation and focal or dif fuse infiltrate by lymphomononuclear cells, plasma cells and eosinophils.

The Study of Stratum Corneum in Diabetes Mellitus

Dried pulverized callus from the sole of diabetic patients showed poor water-uptake (p < 0.001 to < 0.05) and water-loss at different humidities (25% to 100%) and at different temperatures (50 C, 370 C and 50' C), a finding not oberved after ether washing or ether- water washing. The total nitrogen content of the diabetic callus showed no significant variation from the normal controls. The cutaneous insensible perspiration when measured by the unventilated chamber technic of Monash and Blank showed poor diffusive power (p < 0.001).

Naevus Lipomatosus Cutaneous Superficialis

Three female patients aged 30,55 and 4 years had large tumour-like masses, which were progressively increasing in size for the last 4,5 and 4 years respectively. Excision of these lesions and histopathlology revealed mature fat cells in the middle, and lower dermis, interspersed between collagen bundles and surrounding the blood vessels. These findings are characteristic of naevus lipomatosus cutaneous superficials.
